Employee + Spouse: $140.37 per pay period ($280.74/month)
Employee + Child(ren): $120.31 per pay period ($240.63/month)
Employee + Family (Spouse & Child[ren]): $170.54 per pay period ($341.09/month)
Dependent Child: Age 26-30: $330.02 per pay period ($660.04/month)
