The Downtown YourChoice Fitness Center is available to all employees and adult Health Plan members at no cost to you. Several work-site facilities are also available to their respective employees. Each fitness center offers cardio and strength training equipment as well as group fitness classes and personal training.
A Liability Waiver/membership form must be on file in order to gain access to the fitness center and/or utilize group fitness, personal training, etc. Please complete a form for each fitness facility you need access to.
