Diabetes Incentive Program: Mission Control
Mission Control is a voluntary incentive program for Health Plan Members with Diabetes.
Participants are eligible to earn up to $300 for bringing or keeping their A1c at a safe level. An A1c at or below 7.0 is recommended as a reasonable goal for most adults because of the decreased risks of the complications of diabetes. These complications include heart disease, stroke, blindness, neuropathy (nervous system disease), kidney disease and amputation. Every percentage point drop in A1C (for example 8.0 to 7.0) reduces the risk of eye, nerve and kidney disease by 40%. This Program will award Health Bucks to those members whose blood glucose is in recommended range. Health Bucks can also be earned by those members who are at greater risk for diabetic complications to bring their blood glucose into a safe or a more safe level.
Three Categories of Diabetes Control based on A1C values:
In Control - A1C at or below 7.0
At Risk - A1C 7.1 - 8.9
Dangerous - A1C at or above 9.0
Health Bucks Reward
In Control | $300
- An A1C at 7.0 or below automatically earns you an incentive of $300 in Health Bucks
At Risk & Dangerous | $300
Improved A1C of one percentage point earns you an incentive of $300 in Health Bucks
- If your A1C of 7.1 or above has dropped a full percentage point (i.e. 9.0 to 8.0) or more from previous Qualifying year, you can earn $300 in Health Bucks
To determine if your A1C has dropped a full or half percentage point, your new Diabetes Qualifying Form will be compared to your form from the previous year. You must turn in your form by August 31 to be eligible for this program.
